Cloud-Based and Automated School Forms and Reporting System
Cloud-Based and Automated School Forms and Reporting System
For Grades 4 – 12 — Elementary, Junior High, and Senior High

Grades, attendance, and school forms — all in one place.

SMASHED – SFs helps teachers, registrars, and school heads import class rosters from LIS, record grades and attendance, print report cards, and download the official SF2 — without the paperwork.

SMASHED-SFs: School Forms, Simplified - dashboard on laptop, phone, and tablet with Grades and Ratings, Attendance Tracking, Online Enrollment, School Forms and SF7, Employee DTR, and Lesson Plans

Rosters & SF1 Import

Upload your LIS School Form 1 (Elementary, Junior High or Senior High) or a CSV, and review the class list before anything saves. Every change is logged, and LRNs can be hidden on screen with one tap.

Grades, Rankings & Report Cards

Term grade entry per class or subject, rankings and proficiency levels computed for you, and SF9 / SF10 ready to print.

Attendance & the Official SF2

Mark attendance by day, add holidays and semestral or summer breaks, then download the official SF2 form for Elementary, Junior High or Senior High.

Enrollment & Parent Portal

Online enrollment applications, and a Parent / Guardian Portal where families check ratings and sign report cards. Advisers check every access request against the class record first.

Lesson Plans, Tests & Answer Sheets

ILAW lesson plans and an assessment generator using your own AI provider key, plus printable answer sheets you scan with a phone camera — the photos never leave your device.

Time Records & SF7

The Daily Time Record (Form 48) with leave entries, and SF7 personnel records, kept separate from the class attendance.

Insights for Every Role

A Home page for each role shows what needs attention today, and a private written note each term sums up how your class is doing — built from your own records.

School-Wide Reports & Roles

Principals, registrars, and officers roll up every section in one place — Subject Statistics with MPS, DO 15 awards, a consolidated SF7. Access follows each person’s title, such as Assistant Principal or Grade Level Head.

Made for Everyday Use

Pages fit phones, tablets, and desktops, with a light or dark display you can switch any time, and clear guides for each role built into the system.

Security you can verify

Graded A on an independent website security test

On October 10, 2026, www.smashed-sfs.com was scanned with the ImmuniWeb Community Edition Website Security Test and received an A. The scan looks at how the public website is served: its HTTPS setup, security headers, cookies, and third-party content.

  • HTTPS only, with HSTS — browsers keep using the secure connection for a full year.
  • Clickjacking, content-type sniffing, referrer, permissions, and cross-origin policies are all set correctly.
  • A Content Security Policy is enforced.
  • No external content is loaded and no tracking cookies are sent.
  • The web server does not reveal its software version.

This is one automated scan of the public website at a point in time. The report also lists what is still to improve, such as a stricter Content Security Policy and a web application firewall. It sits on top of the protections inside the system — verified accounts, sign-in and sign-up limits, an audit trail — and doesn’t replace a full penetration test. View the full report
